ABSTRACT

Se describe el caso de una mujer de 70 años que consultó por dolor abdominal asociado a pérdida de peso y sudoración nocturna. En el examen físico se destacaban una masa abdominal comprendida entre el epigastrio y el flanco izquierdo, de unos 5 cm de diámetro, duro-elástica, móvil e indolora, y al menos tres adenopatías supraclaviculares bilaterales de 2 cm de diámetro, duras y adheridas a planos profundos. Se realizó una biopsia de la masa abdominal, con lo que se diagnosticó un sarcoma de células dendríticas interdigitantes. Se inició quimioterapia con el esquema CHOP (ciclofosfamida, doxorrubicina, vincristina y prednisona). Falleció luego de completado el primer ciclo del tratamiento, a los seis meses del diagnóstico.


A 70 year-old woman was admitted to our hospital with a 3-month history of abdominal pain, weight loss and night sweats. On physical examination, she presented with a 5 cm diameter abdominal mass extended from epigastrium to the left flank, and at least three bilateral supraclavicular adenopathies. A disseminated interdigitating dendritic cell sarcoma was diagnosed through a biopsy of the abdominal mass. After that, a CHOP regime (cyclophosphamide, doxorubicin, vincristine and prednisone) was iniciated. She died after completion of the first cycle of treatment, six months after diagnosis.

ABSTRACT

Activation of epidermal growth factor receptor (p-EGFR) is one of the triggers in the development of many malignant tumors, and the measurement of Argentophilic Nucleolus Organizer Regions (AgNOR) area is used as a marker of tumor proliferation. Alpha actin smooth muscle (αASM) of peritubular cells (PT) and Vimentin expression are modified during the expansion of the CIS /intratubular germ cell neoplasia (ITGCN) to solid seminoma. We evaluated the expression pEGFR, the AgNOR areas, the expression of αASM in PT and the Vimentin in neoplastic cells from CIS/ ITGCN to solid seminoma. 28 formalin-fixed paraffin-embedded archival tissue blocks of Seminomas from the Departments of Pathology of Clinical Hospital and Ramos Mejía Hospital (CABA, Argentina) were used in this study. pEGFR was expressed in CIS/ ITGCN with membranous pattern (8/9), switching to a cytoplasmic pattern in the solid seminoma (15/19). AgNOR areas of atypical gonocytes were increasing from CIS (3.5±0.3 µ2) to intratubular seminoma (3.8±0.4µ2) (p<.5), until solid seminoma (5.3±0.7 µ2) (p<.01). Increase of AgNOR areas is proportional to the expression of pEGFR. Conclusions: pEGFR was expressed in CIS/ ITGCN with membranous pattern, switching to a cytoplasmic pattern in the solid seminoma. The activation of this receptor could be the first step in the mitogenic signal transduction. AgNOR areas complement diagnosis of testicular CIS, but not differentiate between CIS and intratubular seminoma. The value of AgNOR areas is maximum in the invasive seminoma, and statistically different from intratubular seminoma. The intensity of αASM in PT decreased as the tumor progressed and Vimentin was negative in neoplastic cells.

ABSTRACT

Weanling Sprague-Dawley rats were fed on a choline-deficient diet with hydrogenated vegetable oil and corn oil as lipids develop acute renal failure. Pathogenesis of the latter is controversial and an ischemic mechanism has been proposed. Arachidonic acid derivatives are involved in the regulation of vascular tonus. Vasospasm could be due to an increase in tromboxane A2-mediated vasoconstriction or to a decrease in prostacyclin-induced vasodilatation. Enzymes involved in the synthesis of both compounds are tromboxane A2- and prostacyclin-synthase respectively. The aim of this study was to identify the variable number tandem repeats (VNTR) in the promoter region of prostacyclin synthase gene and verify if there exists a relationship between the occurrence of VNTR in those choline-deficient rats which die because of acute renal failure and those which do not. We verified the presence of the VNTR in the prostacyclin synthase rat gene, but we did not find any difference in the molecular weight of the alleles between experimental and control rats. Renal reparation of the acute kidney injury due to choline deficiency in some rats is not related with differences in VNTR in the promoter region of the prostacyclin synthase gene.

ABSTRACT

La epidermolisis ampollar adquirida (EAA) es una enfermedad autoinmune provocada por autoanticuerpos contra el colágeno tipo VII de la sublámina densa de la dermis. Sus manifestaciones clínicas exigen especial atención para diferenciarla del lupus ampollar (LA), el penfigoide ampollar (PA) y la porfiria cutánea tarda (PCT). La clínica, la histopatología, la inmunofluorescencia directa (IFD) y la inmunofluorescencia indirecta (IFI) con la técnica de salt-split son de gran ayuda para el diagnóstico. El caso de una paciente con EAA y lupus eritematoso sistémico (LES) plantea la discusión sobre las diferencias y semejanzas entre el LA y la EAA. La presencia de mecanismos inmunopatogénicos comunes y la asociación de LES y EAA permitiría suponer que se trata de expresiones clínicas de una misma afección

ABSTRACT

Hemos revisado las biopsias hepáticas de 21 pacientes con insuficiencia renal crónica (IRC), de la Sección Nefrología y Hemodiálisis positivos para el virus de la hepatitis C (HCV), por Elisa II y/o PCR. Eran 14 hombres y 7 mujeres, cuyas edades variaban entre 20 y 65 años (promedio 41 años). El tiempo promedio en diálisis era de 64 meses (entre 12 y 192 meses). Seis pacientes eran además Hbs ag positivos. Fueron biopsiados por presentar elevación persistente de las transaminasas de más de 6 meses. Para graduar las lesiones hepáticas se usó el Indice de Knodell modificado. Todas las biopsias tenían hepatitis crónica, en 2 asociada a cirrosis. Ocho pacientes tenían una hepatitis crónica leve; diez, una hepatitis crónica moderada; y sólo tres pacientes tenían una lesión severa. La fibrosis fue leve en 13 casos, moderada en 6 y 2 tenían cirrosis. El análisis de las lesiones consideradas características de la hepatitis crónica C mostró: nódulos linfoides en 6 casos (29 por ciento), lesión del epitelio ductal en 7 (33 por ciento), y esteatosis in 7 (33 por ciento). Los hallazgos histológicos en nuestros pacientes son similares a los reportados de las poblaciones HCV positivos no IRC.
